HealthPathways is a web-based information portal supporting primary care clinicians to plan patient care through primary, community and secondary health care systems. Each New Zealand region has or is creating its own local dementia pathway. If you are a health professional and do not yet have access to your Regional HealthPathways website, the login pages listed below generally include a contact for the local HealthPathways team, who will provide you with a username and password.
